Construct one packet of clinical information in indicating the following diagnostic-therapeutic continuum for one student. Include the following:

• An evaluation report, specifying areas of strength and deficit. Describe formal and/or informal procedures used to gather speech and language information. This report must be constructed by you and not copied. I do not want a formal evaluation because you would not be expected to know how to put that together yet.

• IEP Goals and Objectives- write 2 goals and 3 objectives for each of the 2 goals. Link each goal to an academic/instructional/curriculum standard area.
• A detailed task analysis with at least 7 steps for 2 of the objectives.
• Clinical intervention plans for 3 consecutive therapy sessions, specifying target behaviors, materials, techniques/procedures and evaluation data.
